I have a complaint/grievance, what do I do?2016-11-12T12:55:32-05:00

If you have a complaint, please:

  1. Submit the complaint either verbally or in writing to the Agency Manager or Supervisor or designee.  If you call after normal business hours, you will be contacted by the Agency Manager on the next business day.
  2. The Agency Manager or Supervisor designee will contact you or your representative and will make every effort to resolve the complaint to your satisfaction. They will document all activities involved with the grievance/complaint/concern, investigation, analysis and resolution. You will be notified of the Agency Manager’s decision within ten (10) days.
  3. If the complaint cannot be resolved to your satisfaction, you may request that the Agency Manager submit your complaint to the Agency’s Governing Body.
  4. Call the State of Pennsylvania Department of Health hotline number 1-800-254-5164, to give your complaint, their number is available 24 hours a day.
What if my care worker gets sick or cannot come in?2016-11-12T12:51:44-05:00

We handle any issues that arise such as scheduling changes, illness and shift cancellations. We will immediately assign another care worker to step in until your regular worker resumes.

Yes, we offer 24 hour care, 7 days a week, 365 days a year. We are very flexible and will accommodate your needs for home care services.

Our employees are covered by comprehensive liability insurance and undergo criminal record checks. They are not only qualified, but are also dependable and trustworthy individuals who respect privacy and maintain confidentiality.

What are my rights if I sign up with Ebenezer Home Care?2016-11-12T12:00:59-05:00

Once you sign up you have the right to fully informed and knowledgeable of all rights and responsibilities before receiving care and to understand that these rights can be exercised at any time. You or your designated representative will get a written copy of all these rights to keep. Here are a few that we can list here, the full list will be in your written copy.

You have the right:

  1. To be informed in advance about care to be furnished and of any changes in the care to be furnished before the change is made.
  2. To be informed of the disciplines that will furnish care and the frequency of visits proposed to be furnished and to know that they are cared for by individuals who are properly trained and competent to perform their duties.
  3. To get the information necessary to give informed consent prior to the start of any procedure or treatment and any changes to be made.
  4. To participate in the development and periodic revision of the plan of care/service.
  5. To access to their your record and confidentiality and privacy of all information contained in the consumer record and of Protected Health Information according to HIPAA, Federal and State requirements.
  6. To get the information necessary to refuse treatment within the confines of the law and to be informed of the consequences.

No, at the moment we offer non-medical care services such as Light Housekeeping, Grooming, Eating and meal preparation, Toileting/Incontinence care, Arranging for transportation to and from appointments, Administering emergency first aid, Providing or arranging for social interaction, Shopping and Companionship and more services.

It is your responsibility:

  1. To provide, to the best of your knowledge, accurate and complete information about:
    • Past and present medical histories.
    • Unexpected changes in your condition.
    • Whether you understands a course of action selected.
  2. To follow the treatment recommended by the particular handling of the case.
  3. To accept consequence of your actions if you refused treatment or do not follow the physician’s orders.
  4. For accruing that the financial obligations of your health care are fulfilled as promptly as possible.
  5. To respect the rights of all staff providing service.
